How To Set Up Your Portal
✅ Step 1: Confirm Your Email
Please verify your email by clicking the link we just sent to you. Didn’t receive it? Check your spam folder, let us know, or contact support at support@md-hq.com.
✅ Step 2: Add Key Info to Your Portal
Include important health information and payment details in your Patient Portal. Please be sure to:
Add a card under the “Invoices & Payments“ tab
Add insurance info (if you’d like to use it for labs, etc.)
Request your appointment through the portal — or call/text us at 512-814-0148
✅ Step 3: Complete Your Forms
All forms are digital (no printing needed!) and must be completed before your first appointment. Find them in your Patient Portal under the “Forms and Questionnaires” tab.
NOTE: Most forms will be made available once your first appointment is booked and confirmed.
